We’re honored that you’re considering Desai Health for your psychiatric medication management and psychotherapy care. Our goal is to make the intake process clear, and efficient so that we can determine how best to support your needs and find the best fit.
Before completing the intake paperwork, please take a moment to review the information below to ensure that our practice is the right fit for you or your child.

Conditions We Are Unable to Accommodate

To ensure that we can provide the highest quality care within the scope of our practice, we are not currently accepting new patients who:
Are seeking treatment for substance use or abuse
Are in treatment for eating disorders or personality disorders
Are involved in custody proceedings or other legal disputes
Are prescribed two or more controlled substances
Require evaluations for disability, Social Security benefits, or legal matters
Are under 8 years old (under 6 for medication management) or over 60 years old

We appreciate your understanding and encourage you to reach out if you have questions about our clinical scope or services.

Insurance Participation

We accept a range of commercial insurance plans; however, coverage may vary by provider. We strongly recommend calling our office to confirm network status before submitting your intake forms.

Please note: We do not accept any Medicaid plans, including Aetna Better Health, United Healthcare Medicaid, CMS, Florida KidCare, or Healthy Kids. We also do not participate in United Healthcare Medicare Advantage or Dual Complete plans.

If you are uninsured or do not plan to file out-of-network benefits, we offer self-pay rates, and will provide a Good Faith Estimate upon request.

Intake Process

Once your completed intake forms are submitted, your information will be reviewed by our clinical team. If we determine that our services are a good match for your needs, we will reach out within 2–3 business days to schedule your first appointment.
